DKA Award Nomination Form

This form is intended for your use in nominating qualified candidates for the David K. Andrews (DKA) Distinguished Alumni Award.

Please use one form per candidate and submit the completed form no later than December 1st in order to allow your nominee(s) to be considered for the current year's award.

To be eligible, nominees must be living alumni of Principia Upper School or College. Although nominees may have worked at Principia, the intent of the award is to recognize alumni who have been of service outside the Principia community.

Nominees must meet the following standards:

  1. Active service in the Cause of Christian Science;
  2. Demonstration of the highest level of business, professional, and personal integrity and ethics;
  3. Distinguished service to Principia and participation in Principia programs;
  4. Distinguished achievement in business or profession; and
  5. Distinguished service to city, state, or nation.

If you have extensive background information or material on your nominee, please send it separately to the Alumni Office, c/o Principia College, Elsah, IL 62028. We may contact you for further information on your nominee, if needed. The entire DKA award process is strictly confidential. Nominees should not know they are being considered.
